Hand in Hand
The Hand in Hand program provides wraparound as an intensive care management program for children 0-6 years of age identified with or at high risk of emotional and/or behavioral challenges. The wraparound facilitator assists the children and their families by offering useful guidance through their services.
Services are:
- Individualized
- Family driven
- Strengths based
- Community based
- Culturally competent
- Team based
Recipients of these services will have:
- Support in building a supportive team and social network
- Assistance with referrals to community programs and services
- Guidance through medical systems, social services, and school systems
- Support from other families having similar experienced
- Networking activities like family support groups
Hand in Hand targets the area of Hood, Johnson, Palo Pinto, Parker, and Tarrant Counties outside of Fort Worth. Lena Pope Home provides services to those residing in Parker County and Johnson County.
Hand in Hand is a federal contract administered by MHMR of Tarrant County and contracts with various agencies to provide Wraparound Services in surrounding counties.
